What is WikiVet?
WikiVet was originally launched in 2007 with the objective of providing a one stop shop to support all veterinary students and graduates in their studies. Since then, the site has grown enormously to accommodate the needs of students to access reliable information which fits well with their existing course material. Now in the UK over 70% of all students are registered users of WikiVet and globally we have over 20,000 student users out of a total of NUMBEROFUSERS users.

Do students really need another website?
Most students rely heavily on web material during their course to support their study and revision. Sites such as Wikipedia or Google searches can be invaluable but most of the information is not peer reviewed and is often written for a non veterinary audience. WikiVet in contrast has been created "by vets for vets" and is specifically designed to support a veterinary curriculum. Feedback from around the world indicates that our content does provide a unique and invaluable resource.

Ten Tips on using WikiVet
# The WikiVet search is really powerful and when used correctly will take you to the information you need quickly and accurately. Notice that as you type in your search term, you will see a drop list of suitable matches.
# We have developed a large collection of Multiple Choice Quizzes in WikiVet which are linked in to key sections of the site. You can also go to a complete list of all the MCQs here.
# An important resource for many students now is good quality educational videos. You can view a wide range of anatomy and clinical videos on the provided by different veterinary schools on the WikiVet Video site.
# WikiVet was a lead partner in developing the Online Veterinary Anatomy Museum. This collaboration involved 15 different veterinary schools developing a virtual museum of some of the best veterinary anatomy resources in the world.
# WikiVet has an invaluable list of standard haematology, biochemistry and physiological parameters for all domestic species which can be accessed here.
# The WikiVet site has a different interface when viewed on a mobile phone or tablet to make access easier. There is also an Android App for WikiVet which you can download from the Google Play store.
# Many students find the Dragster drag and drop applications really useful for learning anatomy. You can see a full list of all Dragster resources available for Anatomy and Histology here. There is also an amazing Online Veterinary Anatomy Museum with over 1,000 digital items which have been donated by many different veterinary schools from around the world.
# We have also developed parts of the site in French and Spanish and produced multilingual podcasts which are available on iTunes.
# There are lots of great sections on WikiVet but everybody has their favourite - mine is the WikiVet Donkey site!

What you can do to support WikiVet®
</imagemap>
There are two main ways that students can help us currently. We want WikiVet to be used as widely as possible throughout the world so as many students as possible can have access to this great content. To do this we need to let students know that WikiVet is here, so we're on the look out for student ambassadors. These ambassadors would act as a point of contact for the WikiVet team in their school, and help us keep the student body informed and motivated. You can find out more information at the WikiVet® Student Ambassadors page.
default Vet Schools map
</imagemap>
We're also looking to create school pages about your school. This will let students and graduates around the world find out more about your school, what the teaching is like and found out who they should contact. If you'd like to help us create a page about your school (after all, who knows it better than you?) then have a look at the help page. If you want to see which schools already have pages and ambassadors, take a look at the vet schools world map.

Introduction
Blood pressure (BP) is a useful measurement to help assess the cardiovascular status of a patient during an anaesthetic, or as a diagnostic test for an underlying cause of ocular changes in cats as a result of systemic high blood pressure. Bear in mind that in a conscious animal, a stress free environment is useful in preventing a rise in blood pressure which is induced by anxiety, particularly in cats. Many agents used during anaesthesia have an affect on the cardiovascular system, and blood pressure in particular so it is important to monitor sequential readings and correct BP changes if necessary. Some surgical procedures such as portosystemic shunt surgery, for example may have a direct effect on blood pressure and BP must be closely monitored throughout the procedure.

Normal Arterial Blood Pressures
*Dogs – Systolic 140, Mean 100, Diastolic 75 mm Hg
*Cats - Systolic 180, Mean 135, Diastolic 100 mm Hg
*Horses - Systolic 110, Mean 90, Diastolic 70 mm Hg
Dogs are subject to breed specific variations in blood pressure reference ranges.

Non Invasive Methods
Doppler Flow Detection
Method:
#Usually measured using the metacarpal and metatarsal artery, although the base of the tail can also be used.
#Once a pulse has been detected, the doppler probe is placed in that position on the limb to check that it is audible and the probe can then be taped to the limb.
#A cuff is selected and placed proximal to the doppler probe and attached to a manometer.
#The cuff is inflated until the pulse is no longer audible on the doppler.
#The cuff is gently released and a reading is taken when the pulse becomes audible again.
*This measures systolic pressure only.
*Cuff width should ideally be 40% of the circumference of the limb chosen for this method to be accurate. The cuff should be applied so that only a small finger can be inserted between the cuff and the leg, to ensure that it is neither too tight nor too loose.
Advantages
*Cheap and available in most practices.
*Useful in hypotensive patients.
Disadvantages
*Not a reliable method in horses.

Oscillometric Method
Method:
#The specialised oscillometric cuff is placed on a distal limb or tail.
#The cuff automatically inflates.
#As it deflates, oscillation in the cuff is detected electronically.
*Measures systolic, diastolic and mean arterial blood pressure as well as pulse rate. The first oscillation of blood flow detected is the systolic pressure, the largest oscillation is equal to the mean pressure, and the oscillations subside at diastolic pressure.
Advantages
*Good in dogs.
*Automatic.
Disadvantages
*Less reliable in cats and hypotensive patients.
*Expensive.

Invasive Methods
Arterial Catheterisation
Method:
#A catheter is placed in a peripheral artery and attached to either an anaeroid manometer or electronic transducer.
#An anaeroid manometer gives a mean arterial blood pressure reading; an electronic transducer gives systolic, mean and diastolic arterial blood pressure readings.
#In dogs, the dorsal pedal artery is commonly used.
#In horses, the facial, transverse facial or metatarsal arteries are usually used.
Advantages
*Continuous measurement.
*Catheterisation allows for additional sampling for blood gas analysis.
Disadvantages
*Invasive
*Requires the correct equipment, which can be expensive.

Central Venous Pressure
Central venous pressure (CVP) is an indicator of blood return to the heart. It indicates adequacy of fluid therapy, heart performance and volume loading of the heart. It is measured by the placement of a central line, usually via the jugular vein. The method then used is that described above for invasive arterial blood pressure measurements.
*Low or falling CVP suggests hypovolemia.
*High or rising CVP suggests volume overload or a failing heart.

The book has a total of 200 cases covering majority small animals but also farm and equine. The cases are well thought out with a brief history and any relevant lab tests (blood work, urinalysis, blood smears, further tests) before a question section to help you diagnose the patient. Some of the cases are fairly straightforward whereas some are more challenging. All reference ranges are included in every case and any abnormal results are in bold, helping you to quickly identify which results you need to focus on. The images are all very clear and are very indicative of what you would see in practise. There is an explanatory note with all images stating where the sample is from, the magnification and staining used.

The answers to all the cases are in the second half of the book. They are very useful; explaining all abnormal findings with extra information about the case, follow up sections and references for further reading. I did find the answers being separate slightly annoying at first as you were constantly flipping back and forth between the two, but did find I was more likely to answer the questions myself before looking them up than if they were at the bottom of the page!

I feel the book is most beneficial to students in the clinical years of their course or general practitioners wanting to brush up on their pathology skills with some of the more advanced cases. It works for study alone or in a group using the questions as a discussion point.

I personally love this book – it has helped me become more confident in interpreting lab results and I have been able to put this into practice on my placements. I took it onto my EMS with me, and both the vets and nurses loved it, and were quizzing each other on the cases! The only downside to the book I really have, is that the small, equine and farm cases are all mixed in together – it is quite frustrating and time consuming if you want to focus on one species as you have to flick though the cases to find the relevant ones. Overall, however, I would definitely recommend it- especially if you are unsure and are wanting more practice interpreting lab results in a case based setting.
Also known as: FMDV — FMD

Introduction
This Apthovirus is a small (25nm) +ss RNA virus, unenveloped and has 7 serotypes, namely Oise (O), 'Allemagne (A), 'C (also German), South African Territories (SAT) 1, 2, and 3 and Asia-1.

This disease affects all cloven-hoofed animals, namely cattle, sheep, goats, pigs, deer, elephants and other wild ruminants such as buffalo and kudu etc. It does NOT afferct the horse. The main presentation of the disease is the formation of vesicles.

The disease is NOTIFIABLE in the UK and any animals with the disease, or in contact with the disease have to be destroyed.

The virus replicates primarily in the upper respiratory tract, tonsils, or upper alimentary tract and there is aerosol excretion during this incubation period. This is then followed by a viremia. Virus targets stratum spinosum of stratified squamous epithelia and mucus membranes and secondary vesicles appear after incubation of 2-14 days. Lesions also affect the feet with cutaneous erosions in interdigital cleft, at coronet and bulbs of heals . These feet lesions often take a long time to heal as secondary bacterial infections may ensue and produce true deep ulcerative dermatitis. In the young, without maternal antibody, virus will localize in the heart, particularly the wall of the left ventricle, resulting in multi-focal necrosis of the myocardium and subsequent death. Persistent infection of cattle can occur in unkeratinized lesions, but subclinical carriers do not usually transfer infection except for subclinical buffalo that can transmit the disease.

FMDV causes loss of condition and productivity but is NOT typically fatal. Aproximately 5% mortality (usually young animals); older animals recover. The virus is highly contagious and spread is by aerosol, direct contact, saliva, infected swill and fomites. Pigs produce 3000 times more aerosol virus than cows, but cows are much more susceptible to infection than pigs.

In 1967 and 2001 there were major outbreaks in the UK. The disease is still widespread in many parts of world especially S. America and far East.

Clinical Signs
Main clinical signs include pyrexia, depression, separation away from the herd, drooling, excess salivation, anorexia pain while eating as well as smacking of the lips. Lameness is also a key clinical sign and in pigs may often be the first noticeable sign as mouth lesions in this specie are less severe.

Lesions in the mouth and on the tongue have the following characteristics depending on age of lesions.
: At 0- 2 days there will be unruptured vesicles.
: At 1-3 days newly ruptured vesicles with adherent epithelia at margins will appear.
: By 3-7 days there will be ruptured vesicles, loss of epithelia, no marked fibrous margin.
: At days 7-10+ open lesions with marked fibrous margin will be present.

Teats on animals that are suckling may also develop vesicles.

In pigs and sheep, lesions are less obvious, but vesicles around nose, mouth, and coronary band are present. Pigs have vesicles on snout, which are quickly traumatised to leave an eroded lesion. Presence of a lesion at coronary band means infection is usually less than a week old. The lesions grow down the claw at a rate of 1mm per week.

In cattle, lesions are seen inside mouth, around muzzle, in the interdigital cleft, around coronary band, and on teats. There will be excessive salivation, anorexia, secondary mastitis and on PM there will be lesions in oesophagus and forestomachs.

Diagnosis
Clinical signs are enough to make a provisional diagnosis. This must be confirmed by ELISA for virus antigen. ELISAs are serotype-specific. This method is soon to be replaced by immunochromatography-bedside ELISA to allow on-farm diagnosis. Virus isolation can also be performed in kidney culture cells, and then serotyped by ELISA. Serology for virus antibody can determine past infection and ELISAs are used to detect subclinical carrier sheep. This cannot be done on vaccinated animals.

RT-PCR has been suggested for on-farm diagnosis, but has flaws such as, RNA is readily degraded by tissue enzymes, RNA must be purified before converting to DNA for PCR, false positives can occur easily by contamination with previously amplified DNA.

Samples should be taken from the vesicle itself and include the vesicle fluid as well as the tissue. These should be placed in transport medium and sent for testing.

Control
Recovered animals show immunity ONLY to the serotype of first exposure, and even this is relatively short-lived, therefore re-exposure to the original serotype after immunity has waned will still result in virus excretion, even without clinical symptoms. Infection by a second serotype will result in clinical disease. It is for these reasons, that vaccination is not practiced in the UK. Further, vaccination would mean a loss of meat export markets.

Prevention in the UK
Imported stock must come from virus-free countries that DO NOT vaccinate. Any meat imported from endemic countries must be de-boned.

ANY sign of lesions in a susceptible animal is NOTIFIABLE to the Divisional Veterinary Officer and local police. Once diagnosis is confirmed, all animals on the premises must be slaughtered and incinerated and the premises fully disinfected. Movement is controlled within a 10-mile radius and follow-up serology must be performed to ensure no spread has taken place on any nearby farms. Ring vaccination with relevant subtype to create a barrier of immune animals (although this was not done in the 2001 outbreak) can be used. There is to be no movement on or off the farm as soon as a case is suspected.

In Endemic Areas disease cannot be prevented by slaughter due to large numbers of carrier stock. Annual inactivated whole virus vaccination using local subtypes is used. This vaccine is inactivated by azuridines, using alhydrogel adjuvant for cows, and oil for pigs. Subunit vaccines are ineffective. The course involves 2 initial injections at 4 months (if dams are vaccinated), followed by boosters every 6-12 months, which induces virus-neutralizing antibodies. Vaccination DOES NOT render meat harmful to consumers, but does affect when it can be exported.

The 50,000th registration was by Bristol University first year veterinary student Ella Briscoe from Godalming. “A friend recommended WikiVet to me to help with revision for histology exams” said Ella, “and I will certainly find it valuable support as I continue through the veterinary study course”. Ella’s current goal, once qualified, is to work overseas helping to provide advanced veterinary care where needed. Ella will also receive a gift of some more traditional hard print reference books from the WikiVet team in recognition of the event.

It is now 9 years since the WikiVet project originated from collaboration between the Royal Veterinary College, University of Cambridge Veterinary School, The Royal “Dick” Veterinary School, Edinburgh and later the University of Nottingham Veterinary School. Its aim remains that of embracing most aspects of the curriculum of the Veterinary Sciences as taught in the UK and enhancing veterinary education through the provision of free online resources to students and practitioners worldwide.

Incorporated into the WikiVet Educational Trust in 2014, WikiVet now has registered users from over 130 countries worldwide, with daily access from new veterinary students through to practicing veterinary surgeons from as far afield as India, South Africa and the USA as well as within the UK.

“When the project first launched, we did not anticipate how widespread the uptake would be,” says Dr Nick Short Head of the e-Media Unit at the Royal Veterinary College, one of the founders of the WikiVet project. “The fact that it is has now been used by many veterinary students, both in UK vet schools and around the world, as a key source of quality veterinary learning material, justifies all the efforts of those engaged in its production over the years” said Nick. “We share Ella’s aims in improving veterinary resources globally”.

Additional to the key role played by volunteers, a commercial wing of WikiVet was established in 2015 to help provide financial support for its maintenance and further expansion. Director of WikiVet Trading Co. Ltd, Graham Dick sees the 50,000th registrant as a milestone from which the future of WikiVet will continue to expand. “The goal of making quality veterinary educational materials readily available to all who study or practice veterinary science is a worthwhile challenge, for which funds as well as goodwill of contributors are critical to the Trust’s sustainability”, said Graham. “We hope that Ella and all other users of WikiVet will continue to find its content helpful in whatever aspect of veterinary study they may undertake.”
