Associated with jet lesions from regurgitant or turbulent flow. Damaged endocardium allows the deposition of calcium and phosphorous.
This may also occur with elevated blood calcium; for example Vitamin D poisoning.

Seen in the left atrium with mitral regugitation and mitral endocardiosis.
